How to Renew San Francisco Health Plan

The steps to renew your SFHP coverage depend on your enrollment program. 


Your Medi-Cal qualification is reviewed annually to ensure ongoing eligibility. The Medi-Cal office carefully assesses and cross-references your case file with their records. Pay close to any correspondence from their office because it’ll inform you of your eligibility status.

Healthy Worker HMO

If you’re a provider of In-Home Support Services (IHSS), IHSS Public Authority, Homebridge, or a temporary exempt employee of the City and County of San Francisco, you may be eligible for the Healthy Workers HMO program. Follow the steps below to renew your coverage: 

  • Watch for your annual renewal notification letter in the mail. The day you’re accepted into the program is called your anniversary date. 

  • Once you receive your renewal notice, make an appointment with a Certified Application Assistor (CAA). Their contact information will be in the letter you get. 

  • Bring your renewal notice and any required documents to your appointment. The CAA can help you complete the renewal process and answer any questions. 

Healthy San Francisco 

If you’re enrolled in Healthy San Francisco, you must renew your coverage yearly to continue receiving care.

You should receive a renewal notice from SFHP indicating the deadline for renewing your coverage. Read it carefully to understand the required actions and any documentation needed for the renewal. Then, complete and submit your renewal form online or by mail. 

Try to renew your coverage at least 30 days before your current enrollment period ends. Otherwise, you might have to reapply for the program if you don’t finish it in time. 

What Documentation Do I Need?

While this depends on your eligibility, you’ll typically need to provide the following information: 

  1. Proof of income - Recent pay stub, tax return, or letter from your employer.
